Outpatient Diagnostic Testing
If you're visiting CHS in Hamilton for our outpatient diagnostic testing, please be sure to do the following before you arrive:

• Consult with your physician to see if there are any pre-testing requirements that you must adhere to. (For example, some tests require you to fast prior to testing.)
  
  
• Remember to bring:
  
  • proof of insurance,
    
  • photo identification (such as a driver's license or U.S. Passport),
    
  • a prescription or any necessary referrals from your physician.

Hamilton Physicians Group (HPG)
If you're a new patient visiting a physician at HPG, please be sure to do the following before you arrive:

• If you've made an appointment far enough in advance, HPG will mail you forms that should be completed prior to your visit. Be sure to bring these completed forms with you for your visit.
  
  
• Ask a family member to see if you have any family medical history.
  
  
• Remember to bring:
  
  • proof of insurance,
    
  • a list of medications to keep us up-to-date,
    
  • any past medical records (if you're a transferring patient).
